At Evolution Health we have streamlined our DOT process. You can book online or call us. We offer same day appointments and are open until 6pm to accommodate drivers schedules.

- Review of your medical history (please bring a list of your medical conditions, surgeries and medications)
- Physical Exam
- Blood Pressure, Height, Weight, Heart rate
- Vision/Hearing test
- UA (non drug test) checking for blood, protein, and glucose
What to Expect:
Appointment typically takes 30 minutes to complete
DOT Physical $150
After completion of the physical exam, the examiner will complete his or her
assessment and complete the required documents.
The provider can certify a driver for
the full two-year period or choose a shorter time period if medically necessary.
Some
health conditions require routine annual monitoring while others may just need a
shortened initial monitoring period while other testing is performed.
A certification card
will be provided to the driver at the conclusion of the visit.
If the provider does not have
enough information at the time of the exam, and approval isn’t possible, the exam may
be suspended while the additional information is gathered.
We will then email a copy of the medical certificate to the DOL and typically should be
updated on your license within 10 days.
